• Arrive
on time to complete registration forms
• At least one parent/guardian must be present at tryouts

Permission form and USAV form must be completed at tryouts
• Health information for medical
form must be completed at tryouts
• Two (2) copies of the athlete’s birth certificate must be handed in at tryouts
The above must be completed in order to participate in tryouts. Please remember a parent must accompany the athlete to tryouts,
along with a copy of your insurance card and two copies of your birth certificate. Please make checks payable to 18 Panel
